# Which back symptoms shouldn't wait?

Some back flares are fierce at breakfast and much calmer by supper. Others keep getting worse or come with a new change in your legs.

## Which signs need urgent care?

Get urgent help for new bladder or bowel trouble, numbness near the groin, or a leg that weakens quickly; a foot that starts dragging is another clear warning sign. That shouldn't wait.

Chills or fever matter more after a recent infection or back procedure, while a hard fall needs quick attention when standing is difficult or soreness keeps rising fast. Those signs aren't for home care.

Don't test these changes with another home exercise. Tell the care team what changed and when it began.

If you're unsure, call a regular medical office before waiting overnight or through a long weekend. You aren't wasting anyone's time.

## When can I make a regular appointment?

Book a regular visit when soreness won't settle, keeps waking you, or travels farther down a leg. It's also time when each flare makes you give up daily work.

The Lancet low back pain series states that for nearly all people with low back pain it is NOT possible to identify a specific nociceptive cause; only a small proportion have a well-understood pathological cause such as vertebral fracture, malignancy or infection. Most new episodes recover quickly, recurrence is common, and in a small proportion the pain becomes persistent and disabling - with initial high pain intensity, psychological distress and pain at multiple body sites raising that risk.

The FDA states verbatim that stem cell products, stromal vascular fraction, umbilical cord blood, Wharton's jelly, amniotic fluid and exosome products have NOT been 'approved for the treatment of any orthopedic condition, such as osteoarthritis, tendonitis, disc disease, tennis elbow, back pain, hip pain, knee pain, neck pain, or shoulder pain.' No stem cell product is approved in the United States for any orthopedic use: the only ones with FDA approval at all are blood-forming cells derived from umbilical cord blood, approved solely for disorders of blood production, and there are currently no FDA-approved exosome products.

The 2025 ASIPP regenerative-therapy guideline for chronic low back pain - written by the specialty society that performs these procedures - grades the evidence as Level III (fair) for intradiscal PRP, Level III (fair) for intradiscal bone marrow concentrate, Level III (fair) for epidural PRP, Level IV (limited) for facet joint PRP and MSC injections, Level IV (limited) for sacroiliac joint PRP, and very low for functional-spine-unit injections. All 19 recommendations are consensus-based rather than evidence-driven, the panel names the scarcity of high-quality studies as the primary limitation, and it states that most of these therapies are not covered by commercial insurance.
